New Patients

We perform an extensive examination of the mouth which includes assessing the teeth, gums and bone support. We also examine muscles, glands and the jaw joint for abnormalities. An oral screening is another important component of the total exam. A minimal number of necessary radiographs will be taken and impressions for study models may be required. A consultation is arranged at which time we will discuss any concerns we have identified. You will be informed of your treatment options and together we will formulate your treatment plan.

This will save us valuable time at the beginning of your appointment. If you are unable to complete the form before your appointment, please arrive 15 minutes before your scheduled time to fill out the form.
